Injury and illness logs (OSHA Form 300) require a listed employer location – what OSHA calls an establishment name – for each entry.

Injury and illness logs (OSHA Form 300) require a listed employer location – what OSHA calls an establishment name – for each entry. This is not per se where the injury occurred, but rather identifying where the employer works or to what part of the organization they belong. Don’t worry, we record the incident location elsewhere.

By default, AWC Retro lists a single employer location for all injuries; the designation is “employer premises”. Some entities will want their injuries to be organized into multiple, specific groups based on facility, department, or other identifier.

Help us give you the report format you need

To help us deliver reports to you in the desired format, we are asking all member entities to complete a very brief survey to identify all employer locations. Those locations/establishments you identify for us will then be an available “employer location” selection when AWC Retro staff records the injury or illness.

Please note that our claims management system generates an OSHA log and summary for each employer location. So, how you want to see your OSHA recordkeeping should determine what employer locations you identify.
