RiskConsole – Retro Staff requesting wages or OSHA information?

One of the benefits of locating your open claims, is that you can enter requested wage information and OSHA required information directly into your employee’s claim.

Here’s how

Login to RiskConsole, go to the Basic My Tab, then select your Member Profile and click on “Claim.” Once in the Claims tab you can view and sort claims. By clicking the “View” button on the left of any claim allows you to open. If you already know the claim number you can also access it on the Basic My Tab, clicking the Quick Search: Claim link.

Requesting info from you

Your Retro Claims Coordinators will only ask for information that is requested by L&I to manage the claim or OSHA required. OSHA requires 18 pieces of information.

Required OSHA information is:

  1. Full name
  2. Address
  3. Date of birth
  4. Date of hire
  5. Gender
  6. Name of physician or health care facility
  7. Address of physician or health care facility
  8. Whether the employee was treated in an emergency room
  9. Whether the employee was hospitalized overnight as an in-patient
  10. Automated case number from the log
  11. Date of injury or illness
  12. Time employee began work
  13. Time of event
  14. What was the employee doing just before the incident occurred?
  15. What happened?
  16. What was the injury or illness?
  17. What object or substance directly harmed the employee?
  18. If the employee died, when did the death occur?
